【摘要】:Recentexperiencewithcomputer-basedpatientrecords(CPRs)hasrevealedmanyvaluablepotentialbenefits,butithasalsobecomeapparentthattheeffectiveapplicationofthistechnologycreatessomenewproblems.CPRsoffertheoptionforlifelonglinkageofallrecordsonapatient,frombirthtodeath.Suchlongitudinalrecordlinkagewouldmakethepatientx2019;sentirepasthealthhistoryretrievable.Thiscouldmakepossibleaquantumleapintheclinicalpracticeofhealthcare,butareliablepatientidentifierisessentialtomakelarge-scaleregionalandnationwiderecordlinkagefeasible.Thedesignofapatientidentifiersystemisnotasimpletask.Incorrectrecordlinkagewouldcreateconfusion,atleast,orpossiblycauseseriousconsequences.Togainthebenefitsfromsuchanidentifier,itmustbeusedbyallrelevantorganizations.Auniversalpatientidentifiersystemmustresistunauthorizedaccesstoconfidentialclinicaldata.Furthermore,thecreationofpersonalidentifiersfortheentirepopulationmustbeacost-effectiveprocessinlightofongoingfiscalconstraints.Thecreationandadministrationofpersonalidentifiersfortheentirepopulationmustbeaccomplishedatacostthatiswidelyacceptedasaffordableandjustified.Last,butnotleast,atimepressureexists.ThesolutiontothepatientidentifierchallengeshouldusetechnologytofacilitaterapiddeploymentofthesystemtopermittheexpeditiousimplementationofCPRs.Acompaniondocument,GuideE2553,providestheimplementationstrategyconcerninghowtoactuallyimplementtheUHIDsystem.1.1Thisguidecoversasetofrequirementsoutliningthepropertiesrequiredtocreateauniversalhealthcareidentifier(UHID)system.UseoftheUHIDisexpectedtoinitiallybefocusedonthepopulationoftheUnitedStatesbutthereisnoinherentlimitationonhowwidelytheseidentifiersmaybeapplied.1.2ThisguidesetsforththefundamentalconsiderationsforaUHIDthatcansupportatleastfourbasicfunctionseffectively:1.2.1Positiveidentificationofpatientswhenclinicalcareisrendered;1.2.2Automatedlinkageofvariouscomputer-basedrecordsonthesamepatientforthecreationoflifelongelectronichealthcarefiles;1.2.3Provisionofamechanismtosupportdatasecurityfortheprotectionofprivilegedclinicalinformation;and1.2.4Theuseoftechnologyforpatientrecordshandlingtokeephealthcareoperatingcostsataminimum.Thisstandarddoesnotpurporttoaddressallofthesafetyconcerns,ifany,associatedwithitsuse.Itistheresponsibilityoftheuserofthisstandardtoestablishappropriatesafetyandhealthpracticesanddeterminetheapplicabilityofregulatorylimitationspriortouse.
